Start at 2.5 mg once weekly and step up by 2.5 mg every 4 weeks as tolerated (2.5 → 5 → 7.5 mg…). Hold at a dose or drop back if side effects climb — there's no prize for moving fast.

Based on a 10 mg vial reconstituted with 2 mL BAC water. The table shows common doses in units on a U-100 syringe.

U-100 syringe: 100 units = 1 mL. At 10mg/2mL, 2.5 mg = 50 units. Past 5 mg a single draw won't fit — re-reconstitute with more water.
Climbing doses

As you titrate past 5 mg, a 10 mg vial at 2 mL gets impractical (>1 syringe). Use a larger vial or more BAC water so each dose stays under 100 units — recompute mg ÷ mL.
